We are looking for grieving moms who feel called to help create a safe and supportive space for other mothers who have experienced the loss of a child. As a support group host, you will have the opportunity to encourage, connect with, and walk alongside moms who understand this unique journey.

In the process, you will also build meaningful friendships and a support network of your own with women who truly understand your heart. BROKEN TO BEAUTIFUL HOST APPLICATION

Support Group Host Volunteer Agreement, Code of Conduct, Medical Authorization, Media Release, and Waiver of Liability

Thank you for your interest in serving as a Broken to Beautiful Support Group Host. Broken to Beautiful, a program of the Schuylar Barnes Foundation, exists to provide grieving mothers with hope, connection, support, and community. As a Support Group Host, you play an important role in helping ensure that no grieving mom has to walk her journey alone.

Please read this document carefully and sign below to indicate your agreement.

Volunteer Service Agreement

I understand that I am serving as a volunteer Support Group Host for Broken to Beautiful and the Schuylar Barnes Foundation. I understand that my role is to facilitate support group meetings, foster meaningful connections among grieving mothers, and represent the mission and values of the organization within my community.

I understand that I am serving in a volunteer capacity and am not an employee, contractor, partner, or agent of the organization. I am not entitled to wages, benefits, workers' compensation, unemployment compensation, or other employment-related benefits.

Eligibility Requirements

By signing this agreement, I certify that:

  • I am at least eighteen (18) years of age.

  • I have experienced the loss of a child.

  • I desire to support and encourage other grieving mothers.

  • I am willing to uphold the mission, values, policies, and Code of Conduct of Broken to Beautiful.

  • I am capable of serving in a responsible, respectful, and compassionate manner.

  • The information I have provided to the organization is true and accurate to the best of my knowledge.

Support Group Host Code of Conduct

As a Broken to Beautiful Support Group Host, I agree to:

Respect and Compassion

  • Treat all participants with kindness, dignity, and respect.

  • Foster a welcoming, supportive, and judgment-free environment.

  • Recognize that every mother's grief journey is unique.

Confidentiality

  • Respect the privacy of all participants.

  • Keep personal stories and information shared during meetings confidential unless disclosure is required by law or necessary to prevent harm to oneself or others.

Appropriate Boundaries

  • Maintain healthy and appropriate relationships with participants.

  • Avoid harassment, discrimination, bullying, intimidation, exploitation, or abusive behavior.

  • Refrain from using my position as a host for personal financial, political, or business gain.

Professional Conduct

  • Conduct meetings in a safe and responsible manner.

  • Communicate respectfully with participants and organization leadership.

  • Promptly notify organization leadership of any concerns involving participant safety or well-being.

Faith-Based Mission

  • Support and uphold the mission and values of Broken to Beautiful.

  • Demonstrate compassion and understanding toward all grieving mothers regardless of where they are in their grief journey.

Social Media and Public Representation

  • Represent Broken to Beautiful in a positive and professional manner.

  • Not speak on behalf of the organization unless specifically authorized.

  • Obtain permission before publicly sharing photos, videos, or personal stories of participants.

Alcohol and Drug-Free Environment

Broken to Beautiful Support Groups are intended to provide a safe and supportive environment for grieving mothers.

The use, possession, distribution, or being under the influence of alcohol, illegal drugs, or controlled substances not prescribed to the individual during any Broken to Beautiful meeting, gathering, training, retreat, or event is strictly prohibited.

As a host, I agree to help maintain an alcohol- and drug-free environment for all participants.

Crisis and Safety Reporting

I understand that I am not acting as a licensed counselor, therapist, psychologist, physician, or legal advisor while serving as a support group host.

If a participant expresses thoughts of self-harm, suicide, harm to others, abuse, neglect, or any situation involving immediate danger, I agree to:

  • Encourage the participant to seek professional assistance immediately.

  • Contact emergency services when appropriate.

  • Notify Broken to Beautiful leadership as soon as reasonably possible.

Medical Authorization and Emergency Release

I understand that Broken to Beautiful and the Schuylar Barnes Foundation do not provide medical, health, or accident insurance coverage for volunteers.

In the event of illness, injury, accident, or medical emergency occurring during my participation in any Broken to Beautiful activity, meeting, training, retreat, or event, I authorize emergency medical treatment deemed necessary by qualified medical personnel if I am unable to provide consent.

I understand that reasonable efforts will be made to contact my designated emergency contact when circumstances permit.

I agree to assume responsibility for any medical expenses incurred as a result of illness, injury, or emergency treatment and release Broken to Beautiful and the Schuylar Barnes Foundation from liability related to the provision or authorization of emergency medical care.

Media Release

I grant permission to Broken to Beautiful and the Schuylar Barnes Foundation to photograph, video record, audio record, or otherwise capture my image, likeness, voice, statements, and testimonials while participating in volunteer activities.

I authorize the organization to use such materials for promotional, educational, fundraising, website, social media, newsletter, and marketing purposes without compensation.

I understand that these materials may be used in print, digital, and electronic formats.

Assumption of Risk and Waiver of Liability

I understand that participation in volunteer activities may involve certain risks, including but not limited to travel, interactions with participants, emotional stress, accidents, injuries, and unforeseen circumstances.

I voluntarily assume all risks associated with my participation as a volunteer.

To the fullest extent permitted by law, I release, waive, and discharge Broken to Beautiful, the Schuylar Barnes Foundation, its officers, directors, employees, volunteers, agents, representatives, successors, and assigns from any claims, liabilities, damages, losses, expenses, or causes of action arising from or related to my volunteer service, except where prohibited by applicable law.

Volunteer Acknowledgment

I have carefully read this Volunteer Agreement, Code of Conduct, Medical Authorization, Media Release, and Waiver of Liability. I understand its contents and voluntarily agree to all terms and conditions contained herein.

I understand that approval as a Broken to Beautiful Support Group Host is at the sole discretion of the organization. Broken to Beautiful reserves the right to approve, deny, suspend, or discontinue a volunteer host role at any time if it determines such action is in the best interest of the organization or the participants it serves.
